Achilles Tendinopathy

Pain and stiffness at the back of your heel first thing in the morning. Aching that eases after a few minutes of walking but returns after a run, a long day on your feet, or a session in the gym. You have tried rest, ice, heel drops — and the pain keeps coming back the moment you build your activity back up.

If this sounds familiar, you are dealing with Achilles tendinopathy — and rest alone is not going to fix it.

What Is Achilles Tendinopathy?

The Achilles tendon is the largest and strongest tendon in the body, connecting the calf muscles to the heel bone and transmitting force during movement. When repeatedly overloaded, the tendon tissue degenerates.

There are two clinically distinct types:

  • Mid-portion: 2–6 cm above the heel
  • Insertional: at the tendon attachment to the heel bone

Clinical evidence shows these respond differently to treatment — accurate diagnosis is essential.

Why Does It Keep Coming Back?

  • Underlying tendon degeneration is not addressed
  • Deconditioning after rest leads to overload when activity resumes
  • Contributing factors are not identified

Common contributing factors:

  • Calf weakness
  • Training load spikes
  • Footwear issues
  • Running biomechanics
  • Sudden activity increases

Clinical guidance confirms that passive rest alone is insufficient — targeted rehabilitation is required.

What Is Achilles Tendinopathy?

The Achilles tendon is the largest and strongest tendon in the body. It connects the calf muscles to the heel bone and transmits enormous force with every step, jump and push off. When it is repeatedly loaded beyond its capacity to recover, the tendon tissue degenerates. There are two distinct presentations of Achilles tendinopathy and the distinction matters clinically. Mid-portion Achilles tendinopathy affects the middle section of the tendon, approximately 2 to 6 centimetres above the heel bone. Insertional Achilles tendinopathy affects the point where the tendon attaches to the heel bone. NICE-reviewed clinical evidence confirms that these two presentations respond differently to treatment — accurate diagnosis at assessment is essential to ensure the correct rehabilitation approach is used.

Why Does Achilles Tendinopathy Keep Coming Back?

Achilles tendinopathy becomes persistent for three reasons, all well documented in musculoskeletal clinical research. First, the underlying tendon degeneration is never directly addressed — pain is managed with rest rather than the tissue quality being improved. Second, when activity resumes after rest, the Achilles tendon is weaker and less conditioned than before. The same loading that caused the problem immediately overloads a deconditioned tendon. Third, the contributing factors are rarely identified — calf weakness, training load spikes, footwear, running biomechanics and sudden increases in activity are among the most common drivers. NICE clinical guidance on persistent musculoskeletal conditions is clear that passive rest alone is insufficient. Active rehabilitation targeting the tendon and its contributing factors is required for lasting recovery.

Shockwave Therapy For Achilles Tendinopathy- What the Evidence Says

Shockwave Therapy for Achilles Tendinopathy

Shockwave therapy has strong clinical evidence for Achilles tendinopathy, particularly mid-portion presentations. Multiple randomised controlled trials and systematic reviews — including evidence reviewed by NICE — have demonstrated significant improvements in pain and function compared to placebo, with results maintained at long-term follow-up.

At Denovo, shockwave is delivered by a certified practitioner following full specialist assessment and always as part of a structured rehabilitation plan. Clinical evidence shows that combining shockwave with a progressive calf loading programme produces significantly better outcomes than either treatment alone.

Shockwave is most effective for:

  • Chronic Achilles tendinopathy present for more than three months
  • Mid-portion presentations not responding to loading programmes
  • Patients who have tried rest and stretching without full resolution
  • Active individuals unable to fully rest from loading activities

Frequently Asked Questions

Mid-portion Achilles tendinopathy affects the middle part of the tendon, causing pain and stiffness. Insertional Achilles tendinopathy occurs where the tendon attaches to the heel bone, leading to similar symptoms but often with tenderness at the heel.
Heel drops can be beneficial for Achilles tendinopathy, but they should be performed under the guidance of a physiotherapist. They help strengthen the tendon and improve its ability to withstand load. However, incorrect technique or premature loading can worsen the condition.
Recovery time for Achilles tendinopathy varies depending on the severity of the condition and individual factors. It can range from a few weeks to several months. Consistent physiotherapy, proper rest, and adherence to a rehabilitation program are essential for optimal recovery.
A scan is not always necessary before your appointment. Our physiotherapists will conduct a thorough assessment to determine the nature and extent of your Achilles tendinopathy. If a scan is required, we will discuss this with you and arrange it accordingly.
